Illinois Public Museum Grants Program Final Report Forms

  OPERATING GRANTS FINAL REPORT FORMS   

Public museums that receive Operating Grants are required to submit a Final Report. A standardized form is provided with your Project Agreement. The report requires an original signature by a designated museum official and is due 30 days after the close of the Project Agreement term for the current fiscal year.

  • The Project Agreement timeline for FY02 operating grants begins March 1, 2002 and closes June 30, 2003.
  • The Final Report is due in the ISM grants office by July 31, 2003.
  • Mail to: Illinois State Museum, Grants Program Office, 502 S Spring St, Springfield IL, 62706-5000.
The Final Report forms are available below in Corel Wordperfect and MSWord formats.
